Hi Kiran,
MIS stands for Management Information System. There is no specific or fixed format for the report. Generally, it is the collection of vital information of all important data. It differs from company to company. In some companies, all major departments are making MIS reports for their department and submit it to the Execution Officer. In some companies, only the HR department prepares and presents it to the highest head.
As MIS reports differ from company to company and department to department, you have to draft it on your own. Identify all the basic needs of a report, whether you need it department-wise or a general one, and determine who is going to prepare it. Check with your concern and design it accordingly.
