Struggling to Find the English Version of the PF A.S.R Form – Can Anyone Help?

Dear Seniors, I have my PF in Chennai. Last month, my PF amount was returned by the banker due to some issue in the bank at that time. Now, the PF office has requested me to submit the A.S.R Form (Advanced Stamped Receipt form) to reauthorize my claim. When I searched the net, I found one A.S.R form, but it is entirely in Hindi. I need the English version of the form so that I can submit it to the Chennai office. Please help me out with this.

I have applied for PF advance, but I didn't notice the bank details while applying. After that, I noticed that the bank details are incorrect. Now, the status of my claim is "Settled but returned." I have no idea what to do. Please, can anyone help me with this?
