Is form numbers change from state to state under factories act? or not?

The Factories Act Forms are one and the Same for all States, may be with slight changes.
Regarding AP Factory Rules, the Forms are available at Law Publico, Koti, Sujatha Law House, Koti,
Hyderabad.
