EPF Application Form for Social Security Number: Clarification and Assistance

Hi everybody,

Recently, the EPF department has made it mandatory for every member of EPF/company covered under PF to fill in a form for the allotment of a Social Security Number. The application form for this purpose is available on their website, but it is not clear and almost unreadable.
