Employees' Attitudes Towards The HR Department

Hi All,
I am sahan HR trainee of a Local Organization in Sri Lanka, These days i have to do a HR audit in our company.
Actually its a leading logistic company in our country, So i want to get the feedback from our employees about the HR Department. Do they get the expected outcomes from us? Are they satisfied with the service we provide? like wise I want to get the existing situation of our HR Department. And then i can anlayse the gap and give the suggestions to the company. And implement the new programme to meet the gap.
So its my pleasure if you all can give me a hand to develop this questionnaire, I think you will pour your valuable ideas & thoughts to help me.
There are more than 200 employees in our organization and i want to get feedback from executive levels and operational levels. Specially from the shared services departments. SO I am waiting for you.
