How to write a letter against an employee to HR department about regular unauthorized absenteeism?

Hello All, Can anyone tell me how to write a letter against an employee to the HR department about regular unauthorized absenteeism after being given two or three chances?

Dear Vijay,
It is a simple letter to be forwarded to HR saying that so and so working as _______________ in our department is abstaining from his / her duties with out proper permission or intimation effective from ______________ till date. Due to his absence the works / production schedules are getting affected hence request you take necessary action.
This is for your information and necessary action pl.,
Pl. attach a xerox copy of his / her attendance card / register.

Dear Vijay,
Hesitation is the first enemy to conquer in order to achieve your growth. After all English is not our mother tongue. But unless U start writing U willn't learn much. If you require assistance now it is only a starting trouble. Many MBAs and IASs are also having such problems. Start writing as Kamesh told, put it on paper. If U desire show to your Srs. and get it corrected that's all.
