Interview Record Sheet - Doc Download

Hi,
Since i am a fresher and i m wurking with a very small start up IT company, so i am trying to build in systems for this company....There is a large amount of recruitment that goes on for this company every week...So, i am trying to chalk out an Interview record sheet which would keep a track of feedback given by the interviewers...and also keep a record of rating of the candidate on the basis of certain parameters like, it skills, communication, personality, etc..
Please help me out in this regard ....since i cant think of more than this...IT wud b of great help if someone could forward me the format of interview record sheet...my id is [Login to view]...
