S. Joyson was right. They will attach one covering letter. Check that out. Most of the time, Form 3A would be wrong. Be clear with the Date of Joining, Date of Resignation, and Form 3A. If you took a single-side Xerox and filled that form, they will reject it. The form should be back-to-back. The employee should sign in 5 places, the employer in 2 places, and the company seal in 2 places. Make sure you have filled all the criteria.
