Dear Friend,
If your PF form is rejected you need to check the reason for rejection. It might be due to various reason. But most commonly PF settlement form are been rejected due to the miss match in form 3A and 6A submitted by the employer or it might be due to miss match in D.O.J... Just reply with the exact reason as for what your PF form was rejected.

S.... Joyson was right.. they will attach one covering letter. check out that. most of the time form 3A would be wrong. clear with Date of Joining, Date of Resigned & form 3A.
if u took single side xerox and filled that form means, they will reject. Form should be back to back. signature of the employee in 5 places, employer sign in 2 places, company seal in 2 places.
