The pdf and Xl file posted years ago are excellent sources for reference and implementation.
I have a suggestion for those who would like to use the Xl format.
The given format when completed will give you an idea of the PRESENT levels.
The trainer should in consultation with the client establish the DESIRED levels.
Say some one on a '1 to 10' scale has scored 3.
It is very unlikely that any training will take the person straight away to 10.
Immediate goal, say for the year would be something like 5.
Over a period of three years or`so, depending on the intensity of training and practice
the person will reach a score of 10.
This will call for two columns for every individual one showing the score "before"
the training and the other one "after" the training.
