How Can I Collect Honest Employee Feedback to Resolve Team Misunderstandings?

I am working as an HR in a software company with 100 employees. I want to collect feedback on every employee from their colleagues. This is very important now because there are misunderstandings between different project teams. Therefore, I want to collect the feedback from the employees first and then try to solve the problem. It is very urgent, seniors. Please try to help me out with a very good format.
