Building a Skills Matrix and Appraisal System in Apparel Manufacturing: Where Do I Start?

I work in the manufacturing and retail sector for apparel. Currently, there is no formal HR system in place. I am looking to establish a gradation pattern and appraisal system and need assistance in creating a skills matrix. Your help on this matter would be greatly appreciated.

Please find enclosed the skill set matrix format for your reference. You can add skill names and create different sheets for various departments. Then, you can ask the respective team managers to fill out the skill set matrix for their departments.
