Plz find a incident below.

IP details:

Name- Sh. ABC Employee, Age- 43 years , LPDT (Low Proficency Dumper truck)Operator, Experience-3 years

Location –315 mRL / near North Ramp Inset

Details of accident :

While operator was driving empty LPDT No.3 from surface to underground (315mRL), LPDT ran over a boulder near 315mRL north ramp junction due to which operator bounced on his seat and his head hit with the canopy. There was no external injury because of the helmet.

Root cause:

Failure in hazard identification..

Careless & unsafe driving.

Recommendations:

Driver should have stop the vehicle and inform the concerned person for cleaning the boulder from the passage.

Driver should not take adventurous activities.



It was suggested that seat belt could have saved the injury.

Kindly suggest how can we provide seat belts as the heavy vechiles dont have & suggest any vendor who can fix seat belts to existing vechiles.Or what can be done to provide seat belt in existing heavy vechlies.

Hi Hansa,
I not currently working in India,however seat belts are mandatory for all heavy equipment in most of the countries.
Again it is very important not to jump out from a vehicle when it is roll over.

Dear Hansa,
First of all thanks for sharing a incident with us. As per my knowledge we shouldn’t modify/alter anything without manufacturer advice it may cause severe injury i.e., seat belt, seat arrangements in car etc..,
What I suggest to you just write a mail to equipment manufacturer they will advice/suggest you it’s the easiest and safest way I ever followed.

as correctly said by Raghu, you need to involve the vendors in this. the vendors will provide you the authorized party details for their equipments.
it was a good sharing & we can also share the same with our employees.
also, it looks like that the driver was not sitting firmly on the seat, as he had bounced from the seat. in the absence of seat belt, i would suggest you to provide more awareness to the drivers about their sitting posture & how to behave incase of any bounce. trust this can give some awareness.
on the other post i read for the procurement policy for the vehicles. in our company we sit, discuss & give our requirement to the procurement. when the same is being ordered, the procurement simply update all the points mentioned by us in the purchase order & give it to the vendor.

Good Morning and thanks for sharing a good learning incident.
Mr. Raghu has rightly said that you should involve the Vendor. We have done this.
Pls note that we have coroprate policy for the transportation of materials / for the employess.
We are the first company in our area (Dahej, Gujarat) rignt from the first day installed seat belt on every seat of the Bus, Tavera, Forklift, Sumo, etc. Vehicles and people are using it correctly.
You must have corporate policy for the Vehicles.
This is for your information and action.

We were in the process of installing seat belts in Hydra(Mobile Crane). We contacted the manufacturer and they just denied our request saying this we will not do. You may install by your own in the internal workshop...

However the best option is always going through the manufactorer. But I not think it's not much essential as it's not a big technical issue. You have to ensure the services of a good maintenance engineer. That's it.

We install seat belts in Tractor, Back Seat of DI Mahendra Jeap, Hydra, etc. In all these you can't expect the manufacturer help.

The purpose is to be solved in right manner. I don't think providing seat belt leads to a big issue.

Let's think in such manner that if anything happening, seat belt will save the life, instead of thinking seat belt will malfunction. Surely contacting manufactorer and they come and install seat belts will taking more time than we going for localized solutions.

It's my view only. Individuals can adopt their practices as per site/locations specific rules.

Thanks for your reply and also i was surprised on your reply. Manufacturer denied our request?? In some case manufacturer may not able to do (Install safety belt) because of company policy etc..,, But they should advise us or refer some vendors for install Safety Belts. In this time I want to highlight something ‘’One notable seat belt failure lawsuit resulted in a $6.5 million judgment against Ford Motor Co.’’



More than 8 million seat belts were implicated in the recall affecting Honda, Mazda, Nissan, Ford, Mitsubishi, Subaru, Isuzu, General Motors, Suzuki, Daihatsu, Geo, GM and Chrysler.(See the Attached).



What I suggest on this matter better we discuss with Manufacturer (Should be on mail) if they are not able to do or not suggest any vendors as far as its on mail its fine then we will fix our own. If something happens in future our law protect us and will help to get claim from manufacturer(As a safety professional I should not say this butttt…////).

Thanks for your participation into the thread and sharing your experience...
I know one case person lay in which one dumper operator saves his life only because of seat belt... The dumper over turned from a ramp of height 13meter and landed at ground floor in tyre up position... But the operator escaped without even scratches....
Keep on sharing and keep up the pace...
