Distribution: One completed copy to the Human Resource Department. The employee may keep one copy.

Apart from that, I don't think you need this format at all when you are using the biometric machine. Even if employees forget the card at home, they can still punch the attendance via the machine.

Which machine are you using? Nowadays, most biometric machines provide multiple systems to mark attendance, such as:
- Punch Card/Swipe Card
- Thumb
- Pin (Just Like ATM Pin)

So, if an employee has forgotten/lost the card, they can still enter the PIN and mark their attendance. I have this kind of machine with me. Now, you can get all the exception reports, like who used the card, who used the PIN, and who used the thumb.

I have created a format for those who forget their ID cards or fail to swipe their attendance at my office. We are utilizing this format in my office, and I hope it proves useful to you as well.

Do you really need formats for this? We just get an email, which the employee addresses to HR and marks a copy to his HOD, giving his IN/OUT time. Generally, he mentions "forgot swipe card." If this is frequent, one can investigate if there is a loss of card, etc.
