Designing a Performance Management System for Telecom: Seeking Advice on Appraisal Formats and Indicators

Hi all, I am working with telecom infrastructure company. I have been asked to design pms for the company. Plz help me with pms booklet preparation…also kindly provide suggestions regarding the appraisal format, the performance indicators to be used, the rating system and the method of calculating the same.
