Please tell me the difference between ESIC Form 12 and ESIC Form 16. When should these forms be used?

No, I think it is wrong. Now, the Form 16 is known as Form 12 as I have seen that both have almost the same format, and Regulation 68 is mentioned in both. Please confirm if I am correct.

You are right, it was before that the Accident Report was Form 16. However, in the ESIC guides, it is still referred to as Form 16. For further information, you can look into the attachments. I hope it helps.
