Creating a Consultant Pay Slip: What Should Be Included and What Can Be Omitted?

Hi Everyone, we are in IT services and consulting. I'm looking for a payslip proforma/format for the consultants we have recruited on contract. Also, I heard that there is no need to include basic salary, HRA, PF, and other allowances for consultants. Please share your views...

I am hiring a consultant who will work with one of my main vendors for about a year or longer. The consultant will be working with my client remotely and supporting client requirements. Now, can you tell me how to create a form or format without displaying any benefits on his/her pay slip? Suggestions and shared formats are highly appreciated!
