Change Of Name In PF Account

What is procedure to change name in PF account and how much time it takes. I have just added last name to my name as there was none previously. I have a notarized affidavit and newspaper cutting with me as a proof of addition of last name. Will that be suffice for proof.

You have to apply through your employer only, they have to agree that your name has been changed and entered in their records.
Attach gazette copy of such name change along with other copies as you have stated.
Submit a fresh Form 2 to avoid consequences in later stage.
Ensure that the addition in the name correctly entered in EPFO records and check the same through your Form 23 also.

Please ask your employer to correct Form 5 as PF office always refer to Form 5 only for such type of cases

NOW I LEAVE THE SERVICE BUT I DONT KNOW WHAT TO DO,PLEASE SUGGEST ME ANY SOLUTION.

My name is Chethan Kumar PR from Bangalore(Karnataka), I am working in DEN Networks Ltd, my question is while submitting documents I have given properly CHETHAN KUMAR PR, but I checked my PF Status through UAN No, its showing CHETAN, so now what is the procedure I can change my name as in documents, firstly I informed my Employer the same Issue.
