How Do I Change My Name in a PF Account and What Proof Do I Need?

What is the procedure to change the name in a PF account and how much time does it take? I have just added a last name to my name as there was none previously. I have a notarized affidavit and a newspaper cutting with me as proof of the addition of the last name. Will that be sufficient as proof?

You have to apply through your employer only. They have to agree that your name has been changed and entered in their records. Please attach a gazette copy of such name change along with other copies as you have stated. Additionally, submit a fresh Form 2 to avoid consequences at a later stage. Ensure that the addition to the name is correctly entered in the EPFO records, and verify the same through your Form 23 as well.

One more point in addition to Sethu: Please ask your employer to correct Form 5, as the PF office always refers to Form 5 only for such types of cases.

I want to know, my name is Roshni Nair in my PF account, but my original spelling is Roshini Vinod Kumar. I'm married now, and in my bank account, my name is Roshini Vinod Kumar Roshini G. Nair. The problem is that my PF consultant wrote my correct name when filling out my PF form, but when my yearly PF statement arrived, my name spelling had changed to Roshni Nair instead of Roshini G. Nair.

I have left the service now, but I am unsure of what to do. Please suggest any solutions or draft a letter for me. I would be very thankful to you.
