How Can We Create an Effective Evaluation Questionnaire for Management Trainees?

Dear All HR Professionals,

Nowadays, many management students join companies for a short period to complete their project as a curriculum part of their course. Similarly, in our company as well, for such management students, the company wants to introduce an evaluation process. I need help in framing the questionnaire. I have attached the questionnaire that will help project trainees understand their project's purpose. I expect inputs for the questionnaire for the Project Guide and anything more.
