How do you assess the effectiveness of your company's existing Performance Evaluation Forms? Do you allow all employees to provide feedback on the forms through questionnaires? Is this feedback valuable?

I found the performance appraisal forms you had posted very helpful. They were very specific and could be measured accurately.
