Eligibile For Gratuity? - Doc Download

I have completed more than 5 years in a company. I have resigned from the company with one month notice.
How can i apply for gratuity?
Which Form i would have to fill ?
To whom i will send this form?
Can i send it via email?
What if the company says you are not eligible because this is not a clause of company?

As i concern you dont need to fill any form to get graduity company will pay it at the time of your clearence.you should concern to HR dept. for this

Dear sharma
You have to send the Gratuity claim form duly filled and signed by you with in one month period after your resignation. I am attaching the claim form for your reference and further course of action at your end.
