I have completed more than 5 years in a company. I have resigned from the company with one month's notice.

Applying for Gratuity

How can I apply for gratuity? Which form would I need to fill out? To whom should I send this form? Can I send it via email? What if the company says I am not eligible because this is not a clause of the company?

As far as I am concerned, you don't need to fill out any form to receive gratuity; the company will pay it at the time of your clearance. You should contact the HR department for this.

You have to send the Gratuity claim form, duly filled and signed by you, within one month after your resignation. I am attaching the claim form for your reference and further course of action at your end.
