Struggling with Unpaid Dues and PF Withdrawal: How Can I Resolve This with My Former Employer?

I am Jameel Pathan, an employee of xxxx Education Ltd. in Chennai. I left xxxx in June as I did not give a one-month notice to the HR department. It has been four months now, and nobody has interacted with me or cleared my outstanding dues. Additionally, they have not sent me the PF withdrawal form. Can I directly send the PF withdrawal form to the PF office to withdraw my contributions? Please guide me.
