Need Help with EPF Withdrawal: Can I Complete Form 3A Myself and How to Get It Attested?

Hi Raj, I worked with a BPO from May 2009 until March 2011, but I was terminated from that company. I am currently trying to withdraw my PF amount independently. However, when I visited the EPF office, they requested Form 3A from my previous employer. Can I download the form and complete it by myself? If so, I would appreciate your assistance with that. Additionally, could you advise me on the attestation process for this form? Your help on this matter would be greatly appreciated.

You cannot download Form 3A. Form 3A is the statement of your monthly contribution details that should be submitted to the PF authority by the employer. Kindly contact your principal employer and ask them to send it from their side.
