How Can We Create an Effective Employee Rating System for Our Reward Program?

I need a format of an employee rating system for a Reward and Recognition program. We would like to recognize our employees promptly and, for that, we would like to rate them on their 360-degree attitude, personal presentation, behavior, performance, etc.
