Hi,
I am a senior HR executive and I have been given the task of building processess in my company. I have drafted a format of a leave card and have attached the same with this message.
I would request all of you to go through it and pass me your suggestions and feedback. Plz note that I work for a media firm.

Hi Karl,
The designed card format is enough for Leave Record.Never forget to update accordingly with emp signature as well as HR signature.Also intimate the staff regarding their Leave balance on regular basis.

Dear Karl,
This card if in hard format must have a duplicate copy, the original with the employer and the duplicate with the employee.
In addition, you may like to insert additional columns to automatically read out bal lve and accumulated lve etc.
You may also like to explore the idea of maintaining registers with all the columns which gives you holistic view of lve records for all the employees.
Link the lve rec to a lve cert to be signed by the line mgr instead of appending the line mgr's signature on the lve card. This will mitigate the need to mov the card around frequently.

Do you plan to accomodate inter- employee comparisons or analysis of leave patterns on parameters like work area, immediate supervisor, whether leaves were availed next to weekends/ paydays/ festivals, whether they were unsched or not

Hi Bharat,
Did really get you. Wat I plan to do is keep a copy with me. I guess keeping one copy with the employee is a good idea. But I am one Person Handling HR activities for 100 employees, so is it practical?
About the other aspect plz mail me some kind of format if possible.

no specific format, just a a sheet of paper , call it the leave requisition,
name
id no
type of lve EL/ CL/ Sick Lve/..........blah blah
from
to
prefix and siffix
lve already availed
signature of employee
signature of line mgr
once this reaches you recommended by the line mgr,
prepare a lve certificate, give one copy to the employee, keep the record of the requisition and another copy of the lve cert with you in a file and use these to post an entry in the lve register.
Now you no longer have the need to maintain lve cards, the register will give you all the details whenever required.

Hi Karl,

I am also working as an HR Executive in a media firm which is into BTL advertising. In this field what I have noticed is that most of the time the employees would be outdoors either on a client meet or on the field. So, keeping a hard copy of the leave card as such may not work out easily especially when you are dealing with a 100 employees.

The way I have done it is I have created a soft copy of the leave file and have asked the employees to put in their leaves via e-mail to their superiors CCing to me.

Formulas can be used in the leave file for each cell. For example when you make an entry in the "leave taken" cell, automatically a digit gets reduced in the leave balance section.

This way it becomes easier to monitor availed leaves for a such a large number of employees in a short period of time. For a hard copy of the record getting the employees in office all the time poses a challenege and can also lead to procastination of leave monitoring resulting in accumulation of work.

I do have a leave request letter format in place, but no leave register.

Your work on Leave Card is good.

My doubts are:

1. whether it is for HR file & Records? or

2. You want your employees to use this every time when they avail leave?!

My suggetion would be that you can try this as your page of Leave records in HR under Personnel records.

If so, pl. delete the columns of Employees signature, HOD's signature etc.,

In case you want for your employees to know their leave on credit, a copy of the above card can be given
