Self apprisal is based on various parameters like targets achieving, safety, quality, customer satisfaction, etc.

If you have a JD you can start writing on your accomplishments for each deliverable listed in your JD.
If no JD, you can make a list of all the tasks you are handling and write on how you have performed each task to its completion and highlight on how your work helps in the overall businesss of your organization . Do not write stories.
