How Do I Create a Payslip for My Cash Salary Without PF and ESI Deductions?

Dear Guiders and Motivators, I am working in a BPO company in an urban area (district place). Until now, I have been paid Rs. 8000 as a monthly salary without any deductions in cash. Please help me prepare a payslip for the above payment.

If you are not receiving a payslip, why do you want to create one on your own? This is not a recommended practice.
