u should go to your regional office of ESI and submit your Declaration form with your following employees name, father name, age, relation with employee and address. and form - 01 for registration. and you have to pay 6 month advance payment through challan.

yes it is necessary 6 months challan to esic in advance and you should have too produce 20 employees Declaration Form - 01, submitt with form - 01 Employer Registration and document as follows,
1st sell & Purchase Bill
MOA
Shop License,
Pan Card of Establishment
Salary register,
Phone Bill
Electricity Bill
