How Should I Draft a Warning Letter for an Employee with Frequent Unapproved Absences?

One of my employees is absent without authorization, though not on a continuous basis. He has been absent for 8 or 9 days each month. Despite receiving an oral warning previously, he has been absent again for three days without providing any formal information to management. Therefore, I am planning to issue a warning letter to him. Could you please advise on the appropriate format for this letter?

Depending upon the misconduct, you can give a show-cause notice and an explanation, and issue a warning letter. Alternatively, you can issue an advisory and a warning letter. A template is attached herewith for your reference and records.
