Warning/Showcause letter - employee absent without unauthorization - 8 or 9 absences each month

Dear Sir, one of my employees is absent without unauthorization but not continuous basis. I mean 8 or 9 absences each month. He has received an oral warning, but he has been absent again for three days without any formal information to management. So i want to write a letter warning letter to him. Please tell me any format.
