How Can I Conduct a Job Satisfaction Survey and What Should It Include?

I am working as an Assistant HR in an organization and have two questions:

1. I wish to conduct a Job Satisfaction survey in my organization to determine employees' current job status. Is this possible?
2. If yes, I would like to know if there is a specific format for the Job Satisfaction survey or the points to be considered in the questionnaire.

Please prepare a questionnaire based on various aspects of HR, such as compensation, benefits, miscellaneous, training, and performance management.

I had prepared a questionnaire some time back in my last organization, and I greatly benefited from it.
