Hi,
Sowmya, the appraisal forms are available in citehr please get it from there. Some of the IT companies are following the two way appraisal method ie both TL or TI and Team members sit together and discuss the performance of their performance and rating it. Some companies one way appraisal method ie only Team Leader or TI can rate the team members performance. I can suggest two way appraising method is better where both the persons can have discussion.
