Seeking Help: What Are the Best Questionnaire Models for Employee Assistance Program Research?

I'm doing my PhD in Employee Assistance Program. I am currently in the stage of preparing research data through questionnaires (completed coursework, etc.). Can anybody help me with different models of questionnaires that will be really useful for me?

Dear Guru, your post is a bit confusing to me, as you say that you are "in the stage of preparing Research Data...." What do you mean by completed coursework? Where are you pursuing your Ph.D., and in what mode? What are your aims, objectives, and hypotheses? Before arriving at these, have you not done the literature survey to see what others have done, how, and why in that way? If you have, did you not come across any questionnaires used by others?
