Dear Bhavya,
Have you studied questionnaire design as a topic? If not kindly see
University of Leeds - Guide to the Design of Questionnaires <link updated to site home> ( Search On Cite | Search On Google ) and more at
Google and
Google
The design depends upon your objectives. May I know why you are asking demographic questions. If you have to seek personal info, it's better to do it in the end. You also should tell the respondents why you are seeking their help; it will also help to improve the response rate if you can tell them how they benefit from investing their time in answering.
I would like you to put your thinking cap and ask questions about why you are using different scales and whether your scales are balanced or skewed. Please also check for changes in the tense of the words used in your scales.
Have a nice day
