The design depends upon your objectives. May I know why you are asking demographic questions? If you have to seek personal information, it's better to do it at the end. You should also inform the respondents why you are seeking their help; this can improve the response rate by explaining the benefits of investing their time in answering.
I encourage you to put on your thinking cap and consider why you are using different scales, ensuring they are balanced and not skewed. Also, check for consistency in the tense of the words used in your scales.
