Struggling with My MSW Fieldwork Report – Can Anyone Share Guidance or Examples?

Hello, I am Gautam Majumder, a 1st-year student of MSW from NSOU. We have been told to do concurrent fieldwork in an NGO and write a report on that. I have no idea about this report. Can anybody help me or guide me to write it? Or if you have this kind of report, can you send me a soft copy (in PDF format) of the same, please?

First, you have to write about your fieldwork, including where you went. Second, you have to write about your organization. Third, you have to write about your experience in the organization. Fourth, you have to write about your learnings. Fifth, you have to write your reflection, which means what you feel about that organization and your feelings. Lastly, your conclusion.
