Designing a Training Feedback Form: Has Anyone Done This and Can Offer Tips?

Hi all, I am to design a training feedback form. Could you please help? Has anybody designed one?

Hi Astha,

You know it's very easy to design a feedback form when you know what your objectives are. I mean it may be a pre-training feedback form or an after-training feedback form or a training effectiveness form. So just define your objectives, and then work on it. Feedback form includes details about the trainer, duration, objectives, subject matter taught, and the technology used for training. You check if all that provided was sufficient. You ask for their suggestions at the end, etc. It covers a lot.
