Salary Slip in excel with all deductions usually done in the company?

Pls could any send the salary slip format in Excel of Rs 12,000 with all deductions usually done in the company.

Firstly the CTC must be decided upon which 40%, 45%, 50% or even max of 60% can be basic.
If employee has to be paid PF, Basic must be bewteen 4000 to 6500/-. any thing above 6500/- has an choice in opting PF or not.
HRA is 50% for Metro and 40% to other cities.
Cost on PF is 12% employee and Employer. Admin charges born by employer 1.4 %.

Ok i am giving you breakup of emploer's PF contribution.
Pls tell me whether it is correct or not according to latest standards.
PF :
Employees contribution - 12 % of basic
Employers contribution - 13.61 %
out of which,
Pension Fund - 8.33 %
PF - 3.67 %
Employees deposit linked Insurence ( EDLI) - 0.50 %
PF admin fee - 1.11 %
EDLI admin fee - 0.10 %
Total - 13.61 %
Pls suggest if any changes.
