Please ask him about the contributions for that particular period, the value additions he has made, response time, innovativeness in the way of doing things, where he feels he needs to improve, and team building, etc. These can be inquired during the performance management system (PMS) review.
Please see the attachment of the performance appraisal that you helped with in your organization. Also, you can discuss this with all the Heads of Departments (HOD) because every department has its unique requirements for work, which you can incorporate into the appraisal form.
