I would request citehr members n specially senior members to help me in developing a questionnaire on KPIs for a infrastructure company. I at least want you to give me a guidelines or suggest me how to prepare a questionnaire.

I have just been tasked to prepare a questionnaire for our organization departments, to identify their KPIs. does anyone have any suggestion on how to start? My dateline is this monday, 30 May.

KPI is Key Performance Indicator.This is required for every assignments for accountibility of individual performance.
For eg.For a HR Executive,KPI will Recruitment indicators i.e.Employee Turnover etc.
Accordingly list out the position in your establishments like.Project Engineer,Civil englineer and discuss with the Project head about their KPI.

I have been tasked to prepare a questionnaire for a disseration on Hospital Information System in Hospital and what are the areas i need to focus during the disseration program. Does anyone have any suggestion on how to start? Please advise me.
Here are the questions which m thinking..
1. What kind of question i have to prepare for healthcare industry in hospital
2. There will be a interaction secession too and departmental posting too.
3. What are the areas i need to brush up before go there.
4. Our quide said that your questionnaire must be covered all aspects in related to all departments in which HIS is used.
Please guide me that which type of questions i must be prepared during this period i can gather more information and its also help me for making my dissertation and viva good.
