HR report for all HR Personnels to track their activity and evaluate them for their productivity?

Hi All, I have a small request - Our company is engaged in providing outsourced services in sales and marketing in timeshare product (i.e. club mahindra) . Now our company is planning to have a weekly report system for all HR Personnels to track their activity and evaluate them for their productivity.
Can anyone please recommend any standard format (Report)we can follow.Please help ...Its kind of urgent guys.
