I think you need to clarify your question a bit because it is very broad. Why are you doing the competency assessment? Is it for selection, career development, etc.? Are you looking for a paper-based method or a software-based method?
My company does competency assessments. We have a software product that helps employees identify their skills gaps. The product allows people to create their own Individual Development Plans, which link learning resources to their skills gaps. Before the product can be used, a job analysis must be conducted to identify the needed competencies for the job.
