Many companies have stopped the bifurcation of leave concept; they have only one leave called the Earn leave - EL.
As the employer and employee have a very dicey image on the type of leave obtained, as few components are carried forward to the next year and at one point it is enchased, depending on the company’s policy.
My company had just recently changed its leave pattern with only one component i.e. EL with 36 days each year where - 31 days is for leave and 5 days for LTA. This had made it much easier for the employees and definitely for the HR's who track leave management.
You can jot down a questioner with some few points like –
1. Giving the above option i.e. having only one component.
2. No of permission in a month and for how many hours (Pl give the option 1hr, 2hr etc)
3. Comp off option
4. An opportunity for the employees to also give some idea with regards to the subject.

2 Bereavement,
3 Paternity
