Facing First-Time Performance Appraisals in Manufacturing: What Challenges Should I Expect and How Can I Prepare?

I am working for a manufacturing company as an HR personnel. I am required to do a performance appraisal which has never been done before. I would like to know what challenges I am likely to face and how best to handle them. I would also like help on how to come up with a self-appraisal form for my seniors.
