How Can We Use Resignation Insights to Prevent Losing Key Employees in the Future?

Friends, I am working on developing HR Policy and Procedure for an organization based in the Middle East. I need some support and ideas as I want to develop a FORM that a Line Manager fills when an employee submits their resignation.

Objectives of the Resignation Form

What I want to cover, as I mentioned in the title, is that the Line Manager should review what went wrong and what needs to be done to avert any future occurrence of losing key human assets. Ideally, I would like to cover:

• LM's assessment of an individual
• LM's assessment of circumstances described by an individual leading to resignation
• LM to clearly categorize the circumstances as either internal or external
• LM's assessment of whether such circumstances are manageable
• If yes, whether they recommend so
• If yes, what will be the impact on other employees

Thank you, Milap. The form you sent is for employee assessments, but what I am looking for is feedback from the Line Manager. I want the Line Manager to appraise HR from his perspective and share what he thinks is going wrong.
