Dear Baneet,
Form 12A under Employees' Provident Fund & Miscellaneous Act is the monthly return of remittance. It is submitted on a monthly basis along with Form 5 (New Joiner Detail), Form 10 (Leaving employees details), Form 2 (Nomination & Declaration), and triplicate copy of the challan.
The monthly return of E.P.F. normally contains the following:
1. Form 2 (for new members during the month)
2. Form 5 (details of new joiners during the month)
3. Form 10 (details of employees who left during the month)
4. Form 12A (details of remittance as per challan)
5. Triplicate copy of the challan (through which monthly contributions have already been deposited on or before the 15th of the following month).
