If the problem is written English - get hold of a good high school grammar book (Raymond Murphy - Elementary and Intermediate) and COMPLETE it first page to the last.
A lot of times, we just collect books and materials and may be read a few pages but seldom do we complete them.
If the problem is spoken English, find someone to practice spoken English everyday and ask him/her to help you with your mistakes. Such a person would be difficult to find out, yes, but would be extremely useful.

English like any other language including your mother tongue can only be learned when you put it in practice.
1st thing is that don't get misguided by English training institutes. There are rarely any good ones.
