You can get all prescribed forms from the P.F. Office in your city. All forms have different purposes, and some of them are as follows:

1. Form No. 9 - all details of the nomination form.
2. Form No. 2 - nomination of an employee.
3. Form No. 5 - monthly details of new employees.
4. Form No. 10 - monthly details of left-out employees.
5. Form No. 12(a) - monthly challan details.
6. Form No. 3-a - yearly details of the employee's contribution.
7. Form No. 6-a - summary of 3-a.
8. Form No. 19 - withdrawal of EPF a/c.
9. Form No. 10(C) - withdrawal of FPF a/c.
10. Form NRS - details of leaves, attendance at the time of withdrawal.

Apart from these, many other forms are also available.
