Employee Motivation Index Tool

Dear All,
I am an MBA student and for my dissertation, I have been asked to develop Employee motivation index tool for a company and implement the same. However, i can see definitions on same, but dont really know how to proceed for developing this tool. Is it questionaire based or what?
Please guide how to design this employee motivation index tool.
