Struggling to Create an Employee Motivation Index Tool for My MBA Project – Any Advice?

I am an MBA student, and for my dissertation, I have been asked to develop an Employee Motivation Index tool for a company and implement the same. However, I can see definitions on the same, but I don't really know how to proceed with developing this tool. Is it questionnaire-based or what? Please guide me on how to design this employee motivation index tool.
