I am looking for Interview assessement form from HR perspective It is needed for one of my clients who is in Software Technology.

I use the simple format from HR point of view.
I am HR consultant and emphasise that this form has a limited use (that is more suited for HR assessment).
But whatever the format, skills of interviewing will always remain supreme.
