Required Letter Format For Increment.

I'm sagar ,
I want a format for asking increment from mu companyside.
I have completed one and half a year in a advertising company, and they have not given any increment to me from the date of joining. I have discussed with my Br. Manager about all this, he said to send mail format for this.
So wanted a format for this . plaese help me to make out this , the letter should be impressive & powerful one.
