Is there any rule book for creating an organogram?

Particularly, I would have a problem with lower-level employees who work for two or more people. How should this be indicated in the organogram? Also, how should the reporting structure and appraisal process for such individuals be managed?

You can always have a dotted line for reporting in a position that has multiple reports and a thick line for direct reporting. You may use the MS Visio tool for this.
