Could someone share an unprotected format of a detailed employee form? I would also like to know all the data that should be collected from employees for office records. What documents should be collected from employees when they join our company?

Full name
Permanent address
Present address
Contact number
Emergency contact number (Relative or friend no.)
Dependents' names and details
Blood group
Educational qualifications, etc.
